超声弹性成像在乳腺良恶性实性结节中的临床诊断评价

摘要    【摘要】 目的:探讨超声弹性成像在乳腺良恶性实性结节中的诊断价值。方法:对笔者所在医院诊治的乳腺肿块患者 150例 170个肿块,行彩色超声弹性成像和二维超声,以术后病理结果为金标准,比较良恶性乳腺肿块的彩色超声弹性成像评分差异。结果:超声弹性成像诊断乳腺良恶性结节的敏感性和特异性分别为 88.68%和 89.07%,准确率为 88.82%,均明显高于二维超声检查,差异有统计学意义( P<0.05) ;乳腺良性结节中超声弹性为( 1.77±0.32),明显低于恶性结节,差异有统计学意义( P<0.05) ;良性结节超声弹性评分≤ 3分占 83.02%,明显高于恶性结节,差异有统计学意义( P<0.05)。结论:乳腺超声弹性成像和弹性评分有助于区分良恶性结节,提高诊断的准确性和降低误诊率。     【关键词】 超声弹性成像 ; 乳腺 ; 恶性 ; 结节  Objective: To investigate the diagnostic value of ultrasound elastography in benign and malignant solid breast nodules. Methods: 150 cases (170 masses) of breast masses in our hospital were treated by color ultrasound elastography and two-dimensional ultrasound. The difference of color ultrasound elastography score between benign and malignant breast masses was compared with the postoperative pathological results as the gold standard. Results: the sensitivity and specificity of ultrasound elastography in the diagnosis of benign and malignant breast nodules were 88.68% and 89.07%, respectively, and the accuracy rate was 88.82%, which were significantly higher than those of two-dimensional ultrasound (P < 0.05); the ultrasonic elasticity of benign breast nodules was (1.77 ± 0.32), which was significantly lower than that of malignant nodules (P < 0.05); The ultrasonic elasticity score of benign nodules ≤ 3 was 83.02%, which was significantly higher than that of malignant nodules (P < 0.05). Conclusion: breast ultrasound elastography and elasticity score are helpful to distinguish benign and malignant nodules, improve the accuracy of diagnosis and reduce the misdiagnosis rate.
