Outdoor air pollution as a possible modifiable risk factor to reduce mortality in post-stroke population

摘要 Outdoorairpollutionisaknownriskfactorformortalityandmorbidity.Thetypeofairpollutantmostreliablyassociatedwithdiseaseisparticulatematter(PM),especiallyfinerparticulatematterthatcanreachdeeperintothelungslikePM_(2.5)(particulatematterdiameter<2.5μm).SomesubpopulationsmaybeparticularlyvulnerabletoPMpollution.Thisreviewfocusesononesubgroup,long-termstrokesurvivors,andtheemergingevidencesuggestingthatsurvivorsofastrokemaybeatahigherriskfromthedeleteriouseffectsofPMpollution.Whilethemechanismsformortalityarestillunderdebate,long-termstrokesurvivorsmaybevulnerabletosimilarmechanismsthatunderliethewell-establishedassociationbetweenPMpollutionandcardiovasculardisease.Thefactthatlong-termstrokesurvivorsofischemic,butnothemorrhagic,strokesappeartobemorevulnerabletotheriskofdeathfromhigherPMpollutionmayalsobolstertheconnectiontoischemicheartdisease.SurvivorsofanischemicstrokemaybemorevulnerabletodyingfromhigherconcentrationsofPMpollutionthanthegeneralpopulation.TheclinicalimplicationsofthisassociationsuggestthatreducedexposuretoPMpollutionmayresultinfewerdeathsamongststrokesurvivors.
