Continuous Flow Left Ventricular Assist Device Therapy:A Focused Review on Optimal Patient Selection and Long-Term Follow-up Using Echocardiography-中国期刊网

摘要 Despitewidespreadawarenessanduseofscientificallyprovenlife-prolongingmedicalanddevice-basedtherapiesoverthelasttwodecades,heartfailureremainsaleadingcauseofmorbidity,mortality,andhealthcareexpenditureintheUnitedStates.Mechanicalcirculatorysupportwithacontinuous-flowleftventricularassistdevice(CF-LVAD),eitherasabridgetohearttransplantationorasdestinationtherapy,isanincreasinglyusedtreatmentmodalityforpatientswithadvancedheartfailuresyndromesthatworsendespitetheirreceivingstandardtherapies.CF-LVADsupportcreatesuniquehemodynamicalterationsthatmustbeunderstoodtoprovideappropriatecareforthesepatientsbeforeandafterimplantation.EchocardiographyisessentialintheevaluationofpatientswhoarebeingconsideredfororaremechanicallysupportedbyCF-LVADs.Hereweprovideafocusedclinicalreviewontheuseofechocardiographyintwomainaspectsoftheevaluationofthesepatients:(a)optimalpatientselectionforCF-LVADsupportand(b)followupassessmentofoptimalpumpfunction.
