简介：摘要目的探讨高龄对于孕妇妊娠结局的影响。方法回顾性统计1990年1月到2008年1月在我院就诊建立孕检表和分娩的35岁以上单胎孕妇219例，以及同期的20-29岁单胎孕妇219例。结果在高龄组中早产，妊高征，子宫肌瘤，胎儿宫内窘迫，足月低体重儿，新生儿窒息的发病率以及剖宫产率均明显高于对照组。结论高龄孕妇的母子合并症及并发症的发生率均高于适龄孕产妇。

简介：Objective:Tovalidatemalnutritionscreeningtoolofnutritionriskindex(NRI)againstpatient-generatedsubjectiveglobalassessment(PG-SGA)asagoldstandardtoolincolorectalcancerpatientsbeforeradiotherapy.Methods:Nutritionalstatusof52volunteercolorectalcancerpatientswithameanageof54.1±16.8yearswhoreferredtoradiotherapycenterwereassessedbyPG-SGA(goldstandardmethod)andNRI.Serumalbuminlevelsofpatientsweredeterminedbycolorimetricmethod.Acontingencytablewasusedtodeterminethesensitivity,specificity,andpredictivevalueoftheNRIinscreeningpatientsatriskofmalnutrition,incomparisonwiththePG-SGAinpatientsbeforeradiotherapy.Results:ThefindingsofPG-SGAandNRIshowedthat52%and45%ofpatientsinourstudyweremoderatelyorseverelymalnourishedrespectively.TheNRIhadasensitivityof66%andaspecificityof60%againstPG-SGA.Thepositivepredictivevaluewas64%andthenegativepredicativevaluewas62%.TheagreementbetweenNRIandPG-SGAwasstatisticallyinsignificant(kappa=0.267;P>0.05).Conclusions:Thefindingsofpresentstudyshowedthattheprevalenceofmalnutritionwashighinpatientswithcolorectalcancer.Moreover,NRImethodhadlowsensitivityandspecificityinassessingnutritionalstatusofpatientswithcancer.Itseemsthatthecombinationofanthropometric,laboratoryparametersandasubjectivescoringsystemmaybehelpfultoolsinscreeningofmalnutritionincancerpatients.

简介：摘要目的提高腹外疝、乳腺、甲状腺手术围手术期抗菌药物使用的合理性。方法筛查我院2014年7-12月腹外疝、乳腺、甲状腺手术共219例，根据卫生部38号文件及《抗菌药物临床应用指导原则》进行回顾性分析。结果预防使用抗菌药物的合格率69%，抗菌药物的使用率40%，药物选择的合理性67%，用药时机合理性61%，预防用药时间≦24小时占63%。结论我院腹外疝、乳腺、甲状腺手术围手术期预防应用抗菌药物存在较多问题,应制定干预措施,严肃处理违规违纪行为,提高抗菌药临床应用的合理性。

简介：AbstractBackground:The purpose of this study was to use the modified Delphi method to identify the influencing factors of health-related quality of life (HRQoL) in patients with unruptured intracranial aneurysms (UIAs) after endovascular treatment.Methods:A modified Delphi method to obtain expert consensus on the content of potential influencing factors of HRQoL in patients with UIAs treated by endovascular intervention was employed. The research team consists of three neuroradiologists and one epidemiologist from Xuanwu Hospital of Capital Medical University. They randomly selected 21 well-known experts in cerebrovascular disease diagnosis and treatment as participating experts. The importance of the indicator is based on the 5-Likert scale. The standard deviation (SD), coefficient of variation (CV), mean (), and minimum and maximum scores of each indicator were calculated. The consistency was described by Kendall coefficient of concordance with a p value < 0.05 indicating that the expert consistency was high.Result:Twenty-one and 18 questionnaires were responded in 2 rounds, with effective response rates of 85.7% and 100.0%, respectively. The average authoritative coefficient (Cr) of all 21 experts was 0.88, familiarity with the indicators (Cs) was 0.82, and the judgment basis of the indicators (Ca) was 0.94. Eventually, the values of arterial puncture hematoma, hyperlipidemia, gender, marital status, and hospitalization for other diseases were lower than 3.5; CV for marital status and gender was higher than 0.35. The Kendall coefficient of concordance in the first round was 0.19 (p < 0.001), and the second round was 0.15 (p < 0.001).Conclusion:In this study, the factors affecting the recovery of HRQoL after endovascular treatment in patients with UIAs were analyzed by the modified Delphi method, which provided a valuable evidence for the clinical management and daily life guidance for UIAs patients.

简介：手足口病（Hand—footandmouthdisease，HFMD）是婴幼儿及学龄前期儿童常见的疾病之一，多数预后良好，HFMD流行时伴随较高的死亡率，该病可引起心肌损害。我市2008年5月份出现该病流行，现对收入我院的219例HFMD患儿进行心肌酶检测并分析其临床意义。

简介：目的探讨腹腔镜在诊断和治疗急腹症中的价值。方法回顾性分析219例急腹症患者腹腔镜诊断和治疗的临床资料。结果219例全部明确诊断，准确率为100％，腹腔镜治疗207例（其中3例辅以小切口完成手术），中转开腹11例，1例在腹腔镜下明确诊断后保守治疗痊愈；术后切口感染3例，无死亡病例。结论腹腔镜可准确诊断急腹症病因，大多数患者可在腹腔镜下完成手术，手术创伤较小。

简介：摘要：目的 分析6年余本院收治的219例儿童白血病的病例资料，探讨赣南地区儿童白血病的流行病学特点。方法 收集 2017年-2023年期间确诊的初诊儿童白血病患儿共 219例临床资料，进行回顾性调查，分析本地区患儿的发病年龄、疾病亚型、首发症状、分子流行病学等特征。结果219例儿童白血病中男124例，女95例，发病年龄以11～15岁最多，2～5岁次之。以急性白血病为主，其中急性淋巴细胞白血病(ALL)145例，占66.21％，ALL分型以B系表达为多；急性髓系白血病(AML))68例，占31.05％,以AML-M2及AML-M3为最多见，混合型急性白血病（MPAL）2例，慢性粒细胞白血病（CML）例3例，幼年型慢性粒单核细胞白血病(JMML)1例。首发临床症状以发热为占多数，约47.49%，多数患者有贫血，部分有出血表现、疼痛等。在急性白血病中，WBC增高者占多数有112例，约52.09%，少部分患者RBC正常/增高或Hgb正常，各占14.42%及15.34%。大部分急性白血病患者PLT减低，约83.72%，少数为正常或增高。白血病儿童正常染色体核型101例，占47.64％，检出异常细胞核型111例，占52.35％。在ALL中融合基因TEL-AMLl 阳性检出率最高，占14.48％，BCR-ABL1及E2A-PBXl阳性各8例，检出率皆为3.77％，MLL-AF4阳性4例，检出率1.88％，其他常见融合基因检出率低。在AML患儿中，融合基因PML-RARA阳性最多，检出率30.88％，AML1-ETO 阳性17例，检出率25％，MLL-AF6阳性4例，检出率5.88％。结论 本院收治的儿童白血病以急性淋巴细胞白血病为主，其次为AML，慢性白血病较少见。男性发病多于女性，发病年龄以11～15岁最多，大部分因发热、贫血、出血表现就诊，其血细胞分析、细胞遗传学及分子生物学特点与国内外文献报道大部分相同。
